NEW YORK CRYO PRICING: Effective July 1, 2024*

The screening, collection, processing, and storage of male reproductive tissue are regulated by the New York State Department of Health (0 CRR-NY 52-8) and the Federal Drug Administration (21 CFR Parts 1270 and 1271), with guidelines provided by the American Society of Reproductive Medicine (ASRM) and the American Association of Tissue Banks (AATB) – which provides both standards for tissue banks and guidance for evaluation of donors. ASRM guidelines and AATB standards are more stringent than the FDA or NYS DOH regulations. New York Cryo follows these more stringent requirements.

CLIENT DEPOSITOR - SEMEN (FOR USE WITH A SEXUALLY INTIMATE PARTNER; CPT 99244, 99214, 89259, 89343, 89320, 89398, 82757) - $1,450    
INCLUDES: 

  1. Consultation – A consultation with the patient and New York Cryo's Medical Director to discuss the process of sperm banking, results of the semen analysis, and required infectious disease blood testing, to answer frequently asked questions regarding the comparative specimen quality, specimen stability over time, and the process of releasing specimens when results.
  2. Physical Examination (PE) – can be performed by the referring physician or by the Medical Director of New York Cryo.
  3. Men’s Health Questionnaire (MHQ) – Documents the patient’s medical history
  4. FDA infectious disease screening
  5. Informed consenting process - reviewing and answering questions about the Stored Specimen Agreement and Consent.
  6. Specimen Processing – Includes basic semen analysis, processing of the specimen with freeze media, and cryopreservation
  7. Post-Thaw Analysis – a required test performed greater than 48 hours after the initial specimen freeze to determine sperm survival after the freeze-thaw process.
  8. Comprehensive semen analysis – Performed on the initial specimen and includes:
    1. Volume - a measure of the amount of seminal fluid
    2. Concentration - the number of sperm in a specified volume of ejaculate
    3. Count - the total number of sperm in the specimen
    4. Motility - how the sperm moves. The test includes both the percent of motile sperm as well as a measure of how fast and in what direction, they move.
    5. Morphology (including “Strict Criteria”) - how the sperm looks. This has been correlated with how the sperm functions.
  1. Viability testing - to assess the functional ability of sperm cells within the semen sample by determining the percentage of live and motile sperm present.
  2. Fructose testing – Performed when there might be an obstruction or absence of the seminal vesicles or ejaculatory ducts. This occurs in the congenital absence of the vas deferens or obstruction due to infection or other issues.
  3. Post-ejaculatory urine (PEU) sperm collection - Performed to collect sperm in patients with retrograde ejaculation, a condition in which semen enters the bladder instead of being expelled through the urethra during ejaculation. Medical conditions or surgeries that affect the nerves or muscles that control the bladder and neck, such as diabetes, multiple sclerosis, and certain surgical procedures, cause more sperm to be found in the post-ejaculate urine than in the antegrade ejaculate.
  4. Images/Videos of the reproductive tissue stored – essential for documentation of what is stored and invaluable for the inseminating laboratory to identify the sperm quality. Having these images often saves the patient thousands of dollars in wasted time and procedures.
  5. The first year of storage for up to ten vials. (multi-year discounts are available).

Note: Blood testing must be performed within three days of the laboratory receiving the specimen. We use LabCorp, which has FDA-approved test kits with reflex NAT testing. New York Cryo does not perform blood testing, and the costs for blood testing are not included in our Fee for sperm banking. It is the Client Depositor's responsibility. Please check with your third-party payer to see if you will incur any out-of-pocket costs for blood testing.

MALE DONOR ELIGIBILITY DETERMINATION (DIRECTED DONOR) FOR USE WITH A KNOWN RECIPIENT OR GESTATIONAL CARRIER (SURROGATE; CPT 99244, 99214 (x2), 89259, 89320, 89398, 82757) - $2,500    

Testing in addition to that required for Client Depositors:

  1. Additional Consultations — An initial consultation to review the process and obtain a history will be held. A second in-person consultation will be held for a physical examination and to obtain a semen specimen. A third consultation will be held to review the laboratory testing results. The initial and third consultations can be done in person or by telehealth. The recipient can be included in any of the three consultations or in a separate consultation.
  2. Additional infectious disease blood testing as required by NYS DOH and the FDA for directed donors.
  3. Genetic counseling and carrier screening – this testing limits the potential for genetic disease from the male and female gamete donor to be passed down to the offspring.
  4. Psychosocial consultation – offers emotional support, helps manage treatment-related stress and anxiety, assures all parties understand the process, and assists in navigating the emotional rollercoaster associated with treatment outcomes.
  5. Storage is separate and is billed at our current rate of $60/month for ten cryovials with a three-month minimum storage.

Note: Blood testing and genetic and/or psychosocial consultations are not billed by New York Cryo and are the donor’s or recipient’s responsibility. Blood testing and completion of FDA questions must be performed at an FDA-approved laboratory within three days of specimen processing and cryopreservation and every six months after specimen storage.

We have the most reasonable sperm banking costs available for secure storage of your specimens. In addition, we are available 24/7 for urgent sperm banking. New York Cryo maintains the highest standards for reproductive tissue banking and is committed to providing the most secure and safe facility for our patients specimens. The Owner, Medical and Laboratory Director, Bruce R. Gilbert, MD,PhD, HCLD is a Urologist specializing in Male Reproductive Medicine. He has developed standards that ensure optimal and secure long-term preservation of specimen quality.

WHAT SETS US APART – AND ABOVE – THE REST!

1. Only one client’s specimen is processed at a time using only ‘single use’ (disposable) supplies. Each specimen is processed with quality control checks at each step. This is crucial to ensure the optimum quality of the stored specimen.

2. One licensed and experienced Medical Technologist (specially trained in Andrology) is assigned to handle a single specimen from the time the specimen is received to the placement of the specimen in the long-term storage tanks. A second Andrologist confirms each and every step. This ensures accurate identification and optimum processing of your specimen.

3. Our standards have been reviewed and approved by the agencies that license and inspect New York Cryo. Namely, the New York State Department of Health (DOH) and The Federal Drug Administration (FDA).

4. Our Standards also require a medical history and specific blood studies for specimen processing and storage at our facility. Specimens are stored in a known infection-free environment.

5. We evaluate each semen specimen for quality. This allows for the assessment of specimen quality and the absence of indicators of infection. This evaluation (semen analysis) also allows the laboratory director to define the best processing and freezing technique to ensure the specimen's optimum survival

6. We pride ourselves on having the highest post-processing specimen quality reported. This means that when the specimens are needed, they will offer the best chance for future fertility.
